Direct Patient Care: Provides spiritual support and counseling to home care patients, families and staff in a culturally competent manner that upholds the Kaiser Permanente Principles of Responsibility. Clinical Chaplain is a member of the interdisciplinary patient care team, using evidence-based outcome-oriented assessment tools to participate in creating patient care plan that includes acuity mapping to define frequency of visits/duration, documents interventions/outcomes and meets the TJC/regulatory guidelines of Hospice/Homecare. Provides spiritual support or visits at the time of death as assessed by patient/family need. Will support the admission / consent process in collaboration with the inter-disciplinary team in the explanation of hospice benefits to the patient and families at leadership request. Supports bereavement care plan to surviving family members for up to 13 months after patient dies; including phone support, in home visitations, bereavement support groups and memorial. Chaplains assist with funeral services for deceased patients when requested and as scheduling allows.
