Responsibilities include: To provide advocacy and outreach services to assigned caseload of youth and their families, in individual or family's home and community To monitor Individual Treatment Plans (ITPs) for all youth and families that supports their capacity for growth and change To establish and set appropriate limits and boundaries as a practitioner in order to assure the safety and protection of the family members and maintain the helping role of the practitioner To provide support and structure to individual and families while monitoring their progress towards completion of service goals To transport individuals in a safe manner in matters related to service delivery To provide on-call emergency coverage as assignedQualificationsHigh School Diploma or equivalent.
