Who needs MDC Visiting Application?
The Michigan Department of Corrections has a special form (CAJ-103) that serves as a Visiting Application, that must be completed by persons who would like to visit an inmate who is held in a Michigan correctional facility. It must be taken into account that the potential visitors may only be an individual who the prisoner includes on their list of Visitors beforehand. The number of visitors including immediate family members mustn't exceed 10 people.
What is the purpose of CAJ-103 Form?
The submission of the fully completed Visiting Application is the only legal way to obtain permission to visit the incarcerated person. If the visitor refuses to provide the required information, it is likely that the request to allow them to visit will be rejected.
Is the CAJ-103 Visiting Application accompanied by any other forms?
When submitting the form, there is a need to attach a minor’s birth certificate or adoption certificate if a minor is a potential visitor. Other visitors generally do not require attaching other supporting documents. However, as has been mentioned, prior to filling PU the CAJ-103 application form, all the persons must be indicated on the inmate’s List of Visitors.
When is the MDC Visiting Application form due?
The CAJ-103 does not a have a particular due date for submission; however, the approval of this application must be obtained before the visit.
How to fill out the Visiting Application?
To be allowed to visit the inmate, the person has to provide the following information:

Visitor’s driver’s license number;

Full name,

Address,

Date of birth,

All the names used previously;

Relationship to the inmate;

Prisoner’s name and number;

Authorization that the given information is true.
There is also a short list of yes/no question that must be answered.
