Direct Deposit Form: What Is It?

A direct deposit form is provided to employees by their hiring company or business. The form is used to allow an employee to automatically have their earnings or salary deposited into their personal savings or checking bank account. This form must be fully completed before an employee can have their paychecks directly deposited. Direct deposit is usually offered from the start of employment with the company.

This form will require personal information about the employee and the bank account. This will allow the employer to access the bank account to deposit the earnings. The employee will need to list their full name and social security number. They may also need to list the name and number for anyone else associated with the bank account. The router and account numbers and the name of the bank for the employee's account is also required.
